Concrete contractor fined $55,000 after worker caught in moving conveyor in Concord, Ont.

A Concord, Ont., concrete forming company has been fined $55,000 after a worker was injured when their clothing became caught in a moving conveyor belt during a cleaning task.

Camp Forming pleaded guilty in Provincial Offences Court in Newmarket to failing to provide proper information, instruction and supervision to protect worker health and safety, according to the Ministry of Labour, Immigration, Training and Skills Development.

The incident occurred on Feb. 28, 2024, at the company’s facility at 71 Creditstone Road in Concord.

A worker was cleaning a roller on a Telebelt telescopic boom conveyor while the belt was still moving, according to the ministry. The worker’s upper body was inside an opening under the main conveyor system.

When the worker attempted to move away from the conveyor, their coverall caught on the moving belt, causing an injury.

A ministry investigation found the worker had not received training on the safe use, care and operation of the Telebelt conveyor system beyond general orientation training provided by the equipment vendor when the conveyor was delivered to the workplace, according to the ministry.

The company was convicted on Sept. 12, 2025, of violating section 25(2)(a) of the Occupational Health and Safety Act. Justice of the Peace Rhonda Shousterman imposed the fine.

The court also imposed a 25 per cent victim fine surcharge as required by the Provincial Offences Act. The surcharge goes to a special provincial government fund to assist victims of crime.
