London, Ont., company fined $100,000 after worker killed in fall while installing satellite dish

A London-based company, Integrated Solutions Inc., has been fined $100,000 following the death of a worker who fell from a roof while installing a satellite dish on a residential property in M’Chigeeng First Nation, Ontario.

The incident occurred on Oct. 21, 2020, when a contracted worker was securing a satellite cable near the bottom of the roof slope. The worker fell from the roof and sustained fatal injuries. An investigation by the Ministry of Labour, Immigration, Training and Skills Development revealed that the worker had not been using fall protection at the time of the incident.

In a ruling on July 29, 2024, the Provincial Offences Court in Gore Bay found Integrated Solutions Inc. guilty of failing to take every reasonable precaution to protect its workers, as required by section 25(2)(h) of the Occupational Health and Safety Act. Justice of the Peace Lori-Ann Toulouse imposed the $100,000 fine following the company’s guilty plea.

Crown Counsel Wes Wilson represented the case, which also resulted in a 25 per cent victim fine surcharge. This surcharge, mandated by the Provincial Offences Act, will be directed to a special provincial fund that supports victims of crime.
